We will send you a statement that will include the facility fee for use of the Surgery Center. Your surgeon and anesthesiologist will bill you separately. Pathology, if you use their service, will also bill you separately.

- We will bill your insurance company directly as a courtesy.
- However, you are responsible for any deductible, co-insurance or balance not paid by your insurance company.
- If you do not have insurance, or if you are having cosmetic surgery, you will be required to pay for your surgery on or before admission unless other arrangements are made with our business office. A price quote will be made available to you based on the information provided by your surgeon's office. The final bill may be higher or lower based on the actual procedure(s) preformed.
- Acceptable forms of payment are cash, cashier's check, credit card, or personal check.
- Worker's Compensation claims require more information. If your surgery is the result of an accident at work, we need some specific information prior to surgery. Please tell us the name of your Workers' Compensation carrier (insurance company), the agent or person to contact at work, your employer's address where the injury occurred, the date and time of your accident, and your claim number.
